Work Permit Information

In Pennsylvania, minors between the ages of 14-18 must obtain a Work Permit. Work Permits cannot be issued to children under the age of 14. Work Permits are issued by the school district in which the student resides. It is only valid for employment within the State of Pennsylvania. The following are the directions to obtain a Work Permit. 

A parent must complete this form: https://bit.ly/WHSworkpermit (The student cannot request or fill in the paperwork). Please complete the sections of the application with a red star. This document will then be automatically sent back to the high school office once you’ve completed it.

  1. You must also send proof of birth date (birth certificate, passport, state-issued ID, or driver’s license) via this Google form. You may drop off a copy at the Middle School, entering through the West side of Warwick Middle School (opposite the main office entrance, around the corner from the large library windows) at 401 Maple St., Lititz, PA 17543, if you prefer.

  2. After the application is received, the permit will be issued, and the student will receive an email to pick up the permit in the MS office. If your student does not attend Warwick, you will receive an email when the permit is completed. It will be mailed, or it can be picked up in the middle school office. Summer hours: Monday-Thursday, 7:30-3:30 pm. The Office is closed on Fridays.

  3. If the student is not present when the work permit is picked up, please have them sign the blue work permit and send a picture of them signing it to sraezer@warwicksd.org

  5. If the student loses his/her work permit, please contact the HS office, and they will reissue another one. There is no need to go through the entire process again.
