The Heart of Lancaster is offering free physicals on July 15 & 29 from 5 - 7 PM for Warwick Athletes.

These sports physicals are for any Warwick student who will be entering grades 7-12 in September 2010 who wants to participate in any of the following sports:

• Junior High Sports - cross country, field hockey, football, wrestling, boys & girls soccer, boys & girls basketball, boys & girls track and field and cheerleading.
• Senior High Sports - cross country, field hockey, football, golf, bowling, swimming, wrestling, baseball, softball, boys & girls lacrosse, boys & girls soccer, boys & girls basketball, boys & girls volleyball, boys & girls tennis, boys & girls track and field, and cheerleading.
  

These are the only dates that free physicals will be offered in the 2010-2011 school year.

Please bring completed paperwork to the physical and enter through the Patient Services Door at the Heart of Lancaster and wear shorts and a t-shirt.

To obtain paperwork, go to the Warwick School District website to download and print the forms needed.  On the main page of the Warwick School District website click on the Athletics tab.  Once on the Athletic page click on the Sports Physicals and Preparticipation tab.  Forms required to turn in are the PIAA CIPPE (Sections 1-4) and the Warwick Extra-Curricular Contract.
